How to Grow Your Practice Without Spending More

Marketing your practice in 2027 looks very different than it did just a few years ago. There used to be one lane: a yellow page ad, maybe a billboard, and word of mouth. Now there is an entire urban sprawl of places people could find you, and you have to show up in the right ones. The good news is that a clear plan makes all the difference, and most of what you need starts completely free.

At our recent masterclass, Josh Wheeler and I walked through what a healthy marketing plan looks like right now, from the organic habits that build your foundation to the automations that protect your time to knowing when paid advertising is the right next move. Here is what we covered.

Organic Marketing Always Comes First

Before you spend a single dollar on ads, your organic reach has to be working. Organic marketing is the shaking hands and kissing babies. It is going to the grand opening of the new business down the street, attending the chamber meeting, introducing yourself as the chiropractor who does the thing. If you do not show up for it, there is no way you are going to get it.

One of the frameworks we love for this comes from the Joyfilled Practice Plan. It breaks your marketing activity into two categories: habits and celebrations. Each one has an inside version and an outside version, and at any given time, you want one of each.

Inside Habits

These are the regular, behind-the-scenes things that keep your practice looking and feeling its best. Restocking your cards, refreshing your signage, decorating your calendar board for the month. Small, consistent actions that move the marketing of your practice forward.

Outside Habits

These get you out into your community on a regular basis. Going to BNI or the chamber meetings, visiting new business owners when they open their doors, making sure your cards are in the local coffee shop. These are the relationship-building activities that keep you top of mind.

Inside Celebrations

Think referral contests, review drives, fundraisers for a local cause, or seasonal events inside your practice walls. There should always be something on the calendar for your existing practice members to engage with.

Outside Celebrations

Health fairs, community events, pop-up appearances, moments where people who have never heard of you can experience what you do. As long as you have one inside habit, one inside celebration, one outside habit, and one outside celebration active at any given time, you win.

Automation and AI Are Not the Same Thing

Before we go further, let us get clear on something that trips people up constantly. Automation and AI are two very different things.

Automation is deterministic. When this happens, this happens, and then this happens. It is one pre-planned path forward. If someone calls your office and you are unavailable, an automated text goes out. That is automation.

AI is generative. It is a flexible conversation that can go in many directions, but it is always moving toward a specific outcome. An AI assistant answering leads on your website might take 300 different turns in the conversation, but it is always moving toward getting that person on your calendar.

Most of what we build for our clients starts with automation. You do not need AI to protect your time and serve your people well. Get your automations solid first.

The Seven SuiteFlo™ Automations

Through our work building Practice Suites for clients, we kept hearing the same request over and over: I just need a button for this. So we built seven of them. These are the seven core automations every practice needs to be ready for 2027.

  • ReviewFlo™: One button to request a review at the right moment. Five-star reviews go to Google; anything less comes back to you privately so you can make it right.
  • CallFlo™: A voicemail message funnel that directs callers based on their need: new members, current members, or general inquiries, plus an automated missed-call text-back so no one slips through.
  • LeadFlo™: A dedicated page and nurturing sequence for new prospects, so they experience your culture before they ever walk through your door.
  • RecallFlo™: One button to re-engage practice members who have fallen off your schedule, with a simple one-click response option for them.
  • CareFlo™: Appointment reminders, visit-one confirmations, and post-visit follow-ups, automated so every new member feels cared for from day one.
  • RetentionFlo™: A loyalty program for your most committed members, complete with a landing page, instant sign-up, and automatic tagging in your system.
  • ReferralFlo™: A system for your community referral partners, with branded materials, a personalized landing page, and trackable QR codes so you always know who is sending people your way.

These automations happen behind the scenes. They protect your time and serve your people without you having to manually remember every step. And none of it is AI. It is just one click, every time.

When It Is Time to Pay for Marketing

Here is the honest truth: there is a ceiling on organic reach. At some point, you have done the networking, you have the automations running, and growth starts to plateau. That is the signal that it may be time to bring in paid marketing, whether that is Facebook and Instagram ads, Google ads, or SEO.

Josh Wheeler, our ads and SEO partner, shared this framework for thinking about it:

  • If you want to maintain where you are: Plan to invest about 8% of your revenue back into marketing.
  • If you want to grow aggressively: That number climbs to 12 to 18%. For every $100,000 in revenue, some practices reinvest $18,000 or more to fuel that growth.

Paid ads are not a magic bullet, and they are not where you start. But when your organic foundation is solid and you need to add fuel to the fire, that is when ads become a powerful multiplier of everything you have already built.

The Difference Between Ads That Work and Ads That Do Not

The biggest mistake people make with paid advertising is being too generic. “Do you have knee pain?” will get ignored. What works is speaking directly to the specific person with the specific problem, and understanding what that problem actually costs them.

Someone who does CrossFit wants to get out of pain so they can get back in the gym. A golfer wants to protect their handicap. A mom wants to know she is doing everything she can to keep her family healthy. Same knee. Completely different ads.

Josh uses four formats to find what resonates: a first-person question, a third-person question, a first-person statement, and a third-person statement. People respond differently to each, so you test until you find the message that clicks.

As a starting point, plan on spending around $30 per day per campaign. Give it time. An ad that does not work after 10 days and $300 spent has not failed. It has not found its message yet. Keep refining it.

Know What Makes You Different

Before any of this works, whether organic, automated, or paid, you need to know what makes you the person to call. People do not come to you because you are a chiropractor. They come to you because you are you. That is your most powerful marketing asset.

Build what Josh calls a five-star service stack: a clear picture of the things you do inside your practice that make the experience different. It does not always have to be something no one else offers. Sometimes it is just something people do not know you offer, and once they hear it, they are sold.

If you are not sure what your thing is, ask the people who know you best. Ask your team. Ask a longtime practice member. Ask your mom. The things that make you unique are often invisible to you because you do them every single day.

Lead With Certainty

One more thing, and this one is important. People will follow your lead wherever you set the bar. If you are uncertain about the value of what you offer, in how you talk, in your marketing, in how you answer the question “how long will I need to come see you?” they will feel that uncertainty and hesitate.

Show up with confidence. Speak clearly about what you know you can do for people. The amount someone is willing to invest in their care is in direct proportion to the confidence they have in your ability to deliver. Set the bar high. Hold it there. They will meet you there.

Start With One Thing

You do not have to overhaul everything at once. Pick one area where growth feels stuck, whether that is your outside habits, your RecallFlo™, or your online visibility, and build that first.

A clear plan, executed consistently, will always outperform a complicated strategy that never gets started. If you want help building a marketing plan that fits where your practice is right now, visit stafflesspractice.com or send a text to 908-224-6685 and let’s figure it out together.
